NetScape and Elm Configuration

Hints for Viewing .dvi and .ps Files, by Jon Lee.
The hypertext on this page will frequently prompt you to download a template file. However, files with extensions .dvi and .ps can be viewed from an external window instead. Follow Jon's instructions to configure NetScape to spawn the Xdvi or Ghostview applications for the given examples.

Letters and Mailing Labels in LaTeX

LaTeX format UK letterhead with the official UK logo.

This is an improved version of the letterhead style I wrote for the Department of Statistics several years ago. If you have been using a copy of that format, you may want to download this version and see how you like it. One of the improvements is the absence or less frequent occurences of annoying TeX warnings like "Overfull \hbox" and such. I've also appended a line for your URL.

fax.tex
A personalized fax coversheet that reads your let_head.sty.

There is no need to edit this file each time you produce a coversheet. After you type "latex fax", you will be prompted to \@typein the following information: "TO", "FROM", "PAGES TO FOLLOW" and "COMMENTS".

label.tex

this file is used in conjunction with an input file of LaTeX formatted addresses
to print labels for a bulk mailing.
I've tinkered with this file to fit the labels I have in stock (Avery #5331, 30 labels per page in 3 columns, 10 rows), but you can easily format it for other label styles. A better looking label is the Avery #5260, also a 3-column, 10-row format, but with rounded corners, and designated for laser printers.

Business Cards in LaTeX

This LaTeX file works beautifully with the white perforatted business card template available from

The template prints 10 cards in 2-column format. The card stock is light and flexible enough to feed through the standard paper tray of most laser printers, or you can use the your printer's paper tray by-pass feature. Other business card templates are available with printed backgrounds and grahics, and most are available with matching letterhead, envelopes and postcard templates. I have a sample kit of PaperDirect's products in my office that you are welcome to peruse. Similar products are available at Kennedy's Book Store and Schumaker's Art Supply in Lexington, or you might try some of the local office supply stores.


Dissertation Cover Pages and Forms in LaTeX

Note: Replace the word "Dissertation" with "Thesis" if needed. It appears in several forms of capitalization.

  • Graduate School Administrative Forms in LaTeX

    The hypertext below will prompt you to download a LaTeX version of forms used by the Department of Mathematics and the Graduate School. You can fill-in the information in the spaces provided, and then print the file. Approval signatures are required, and a copy must be left with Mike for your student file. It may be easier, once the file is printed, to leave the completed form with Mike for approval signatures, copies and submission to The Graduate School. Mike will be happy to help you with room reservations for examinations. You should also see the LaTeX template for thesis and dissertation cover pages archived at the Help page available at the UK Math Page. 

    Applying for the Master's Degree

    To be eligible for a degree, students must file an application in the Graduate School within 30 days after the beginning of the semester, or 15 days into the Eight-Week Summer Session, in which they expect to complete their work. Applications for degrees are valid for one year only.
    Final Examination Recommendation
  • ms_final.tex, download the LaTeX template;
  • ms_final.dvi, a finished .dvi sample.
  • This form must be submitted at least two weeks prior to the examination date. The final exam must be scheduled to occur not later than eight days before the last day of classes.
    Thesis Approval Sheet
  • app_thes.tex, download the LaTeX template;
  • app_thes.dvi, a finished .dvi sample.
  • If you are writing a thesis, this form must accompany the Final Examination Recommendation. You must also submit a copy of the thesis on 100% rag or cotton paper accompanied by a letter from the Director of Graduate Studies stating that the copy is acceptable to the program. When the sample is approved they will be filed in the Graduate School and matched against the final copies presented.

    Applying for the Doctoral Degree

    The application for receiving the degree must be initiated by the student according to the deadlines published in the University Calendar. Degree applications are valid for one year only.
    Foreign Language Component Form
  • language.tex dowload the LaTeX template;
  • language.dvi, a finished .dvi sample.
  • In order to satisfy the Foreign Language Requirement, the student must do both of the following:
    1. Satisfy the Foreign Language Requirement described under ``General Requirements for all Master's Degrees'' in the Graduate School Bulletin (pg. 19 of 1994-1996 edition).
    2. Give a seminar talk on a mathematical research paper, which is written in the same language used in 1. above.
    A general notification of the exam is distributed to all persons in the Department of Mathematics two weeks prior to the scheduled date of the exam. Page 1 of the form above is the approval form which requires the signature of your Committee Chair following the presentation. Page 2 is a suggested format for announcing your exam.
    Advisory Committee Request Form
  • adv_comm.tex, download the LaTeX template;
  • adv_comm.dvi, a finished .dvi sample.
  • The Advisory Committee should be formed and appointed no later than the point at which 18 credit hours of graduate work have been accumulated and at least one year prior to the Qualifying Examination.
    Recommendation for Qualifying Examination
  • qua_exam.tex, download the LaTeX template;
  • qua_exam.dvi, a finished .dvi sample.
  • This form must be submitted at least two weeks prior to the date of the examination. Students are admitted to candidacy after they have successfully completed the Qualifying Examination. The Registrar is notified by the Graduate School and the date is noted in the transcript.
    Notification of Intent to Schedule a Final Examination
  • intent.tex, download the LaTeX template;
  • intent.dvi, a finished .dvi sample.
  • This form must be submitted at least thirty days before the exam is to be given. The Dean of The Graduate School will assign an Outside Examiner.
    Request for Final Doctoral Examination
  • ph_final.tex, download the LaTeX template;
  • ph_final.dvi, a finished .dvi sample.
  • This form must be submitted at least two weeks before the exam is to be given.
    Dissertation Approval Sheet
  • app_diss.tex, download the LaTeX template;
  • app_diss.dvi, a finished .dvi sample.
  • The approval sheet accompanies the Request for Final Exam and must be signed by the majority of the members of the Advisory Committee. A copy of the dissertation also accompanies the Request for Final Exam. This pre-exam copy should include:
    A copy should be presented to the Outside Examiner after approval by The Graduate School. After successful completion of the final defense, the student has 60 days to submit a final copy of the dissertation to The Graduate School. Otherwise, a second Final Exam must be scheduled.
