If you expect to or have missed a lecture or recitation class meeting, you may apply for the absence to be excused here. Absences reported in advance are usually excused. Up to 5 (single day) absences reported on this form within 24 hours of the absence are usually excused. If you miss more than five days of lecture, and wish to have these absences be excused, then you should have very clear written documentation that proves these absences must be excused under University Senate Rule 5.2.4.2, with the following clarifications: Our deadline for reporting an (upcoming) absence for a major religious observations is Wed Feb 4th, 11:59pm. Our deadline for reporting scheduled university trips is two weeks prior to the first day of absence. Our deadline for receiving written documentation is one week from the first day of return. Our requirements for “serious illness” are a signed doctor's note with a “return to work” date and a phone number where we can verify the authenticity of the note; if you use university health services this is a “Tier 3” note. Death or serious illness of a family member requires similar verifiable documentation.
